Prevalence and Distribution of Autoimmune-Related Oral Mucosal Lesions and Its Management at the Central Referral and Teaching Hospital in West Java, Indonesia
Introduction: Autoimmune diseases are characterized by dysregulation of the
immune system which causes antibodies to attack their structures. The exact etiology
or pathogenesis of autoimmune diseases is still unknown. Many autoimmune diseases
show early signs of their appearance in the oral cavity. The purpose of this study was
to determine the prevalence and management of autoimmune-related oral diseases at
the referral center hospital of West Java, Indonesia from 2016-2020. Methods: A
retrospective study by observing secondary data from medical records of patients
with oral diseases related to autoimmune diseases at a referral center hospital in
West Java, Indonesia from 2016-2020. Results: From the total of 2,215 patients who
visited the Oral Disease Polyclinic from 2016-2020, as many as 156 (7.04%) patients
had manifestations of autoimmune diseases in the oral cavity. The ratio of female and
male patients was 66.89:33.11%. The frequency of autoimmune diseases was most
common in adults, range 26-45 years of age. The most common oral diseases related
to autoimmune were systemic lupus erythematosus, pemphigus vulgaris, and oral
lichen planus. Managements that were given to patients with oral disease related to
autoimmune disease in cooperation with the Dermatology and Venereology
Polyclinic. The drugs given were corticosteroids, immunosuppressants, topical
antiseptics, topical anesthetics, antifungals, supplements, antibiotics, proton pump
inhibitors, non-steroidal anti-inflammatory drugs, antifibrinolytics, antivirals,
antihistamines, and other topical drugs. Conclusion: The prevalence of oral disease
patients related to autoimmune diseases has increased since 2018, then decreased
until 2020. The most common oral disease associated with autoimmune diseases was
systemic lupus erythematosus, and the most common type of drug given was topical
corticosteroids. Dentists have an important role for early detection and give an
appropriate management which required cooperation with the medical team.
